Volunteer Services Application Form

Welcome new prospective volunteer or re-certifying volunteer. We appreciate your interest in volunteering with the Georgia Department of Corrections (GDC).

The initial step of the volunteer application process is to answer some pre-screening volunteer qualification questions. Honestly answering these screening questions will assist the volunteers services team in determining your eligibility to continue with the volunteer application process based on GDC’s policies and procedures.

You will be required to upload a copy of your driver’s license, or a state issued ID, (with photo) in a PDF/JPG/PNG formats. For ease with uploading you may want to have these documents already saved on your computer.

I understand/ agree to the following Terms:

I am at least 18 years of age.

I understand that I cannot have a family member or be a close friend or associate of an offender in the same facility in which I intend to volunteer.

I understand that I cannot be on an offender’s visitor list in the same facility in which I intend to volunteer.

If I am an ex-offender, I have been completely off probation or parole at least 3 full years.

I understand that as a volunteer, I will not perform professional services unless certified or licensed to do so under the direction of GDC authority.

Represent in an official or unofficial way, or be a member of, an organization identified as a “hate group” by the Office of Professional Standards
or otherwise espousing an ideology of discrimination or violence toward others based upon an
individual’s race, gender, religion, national origin, or other identity.

I understand that as a volunteer, I will not represent in an official or unofficial way, or be a member of, an organization supporting an ideology of discrimination or violence toward others based upon an individual’s race, gender, religion, national origin, or other identity.

I understand that I cannot be a registered sex offender or listed on a sex offender registry to be a GDC volunteer.
